Senior Medical Director for Medicare Serious Illne

Requirements

• MD (Medical Doctor)or DO (Doctor of Osteopathic Medicine)
• Palliative Care or Geriatric experience a plus.
• Experience with Medicare and/or Medicare alternative payment model (APM) preferred.
• and/or Utilization Management (UM).
• Quality Management
• Disease Management (DM)
• Two (2) or more years of experience in Complex Case Management (CM)
• clinical practice and health care industry.
• Three (3) - Five (5) years of clinical experience in the health care delivery system e.g.
• Board Certification (National Board of Medical Examiners) in Geriatrics
• Internal Medicine
• Hospice and Palliative Care
• and EHR preferred

• Additional training or experience in Health IT
• Strong ability to work in and affect change in a highly matrixed environment
• Ability to work independently and adapt to team approach as necessary
