Requirements

• 3-5 years of nursing experience. Leadership experience preferred. 3-5 years care management experience preferably in Home Health Ambulatory Care Community Public Health Case Management and Coordinating Care across multiple settings and with multiple provi
• Must have a current Indiana state license as an RN
• Masters degree in healthcare related field or MSN preferred.
• Must have a BSN from accredited school of nursing. Acceptable accrediting agencies are Commission on Collegiate Education in Nursing (CCNE) Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA).
