The Clinical Triage Nurse responds to incoming telephone calls and other media contacts to assess, provide telephonic medical advice, consultation, referrals, and assistance as authorized in accordance with standard symptom-based guidelines and procedures in a 24/7 call center and virtual care environment. Independently triages calls based on the customer medical condition, symptoms, and care requirements. This level of remote care requires exceptional listening skills and extensive Registered Nurse experience. RN Scheduling/Triage Nurse communicates frequently and effectively with co-workers, physicians, patients, caregivers and vendors. Utilizes EPIC Hyperspace and Remote Client for referral triage and entry. Maintains awareness of supply needs of patients, resources available for supplies and ensures that appropriate levels are readily available to nursing staff for patient visits while practicing good stewardship by looking to keep costs at a minimum. Will be back up scheduler to maintains day-to-day continuity of quality patient care by performing duties of scheduling of daily patient visits in the home setting including, but not limited to: general skilled nurse visits to patients with varying levels of acuity and needs, admission, post-hospitalization, recertification, discharge and supervisory nurse visits.

ACLS certification preferred. 2 years experience in a medical setting required-specialty preferred. Medical terminology and pharmacology background recommended. This position will act as part of the dual eligible member's care team at the state level responsible for integrating and coordinating care to meet the uniquely complex healthcare needs of our DSNP members. This position will assist our dually eligible members with maintaining Medicaid and LIS eligibility and obtaining other Medicaid entitlements available to them. They will be responsible for monitoring member statuses and reporting results to management. The employee must be able to work alongside other DSNP care team members including social workers, care managers and care coordinators to provide the member experience by: • Educating and assisting members on various elements of Medicaid entitlements, benefit plan information and available services created to enhance the overall member experience with the company. • Utilize all relevant information to effectively influence member engagement. • Initiating contact with members who have lost Medicaid eligibility and fall into DSNP grace period to obtain Medicaid and LIS certification. • Assist members with finding resources to help them reapply if necessary. • Coordinating annual reminders for members at risk of losing LIS, Medicaid or DSNP eligibility. • Taking ownership of each customer contact to anticipate customer needs, resolve their issues and connect them with additional services as appropriate. • Identifying member needs beyond the initial inquiry by answering the unasked questions and asking probing questions to identify member needs.
