An exciting opportunity has arisen to work within the Somerset Hub for Coordinating Care (formally Somerset Primary Link) team.
The SHCC team is fast-paced which consists of nurses and admin staff, due to the current COVID climate and an extended Bed Coordination hub this offers new challenges and therefore we are looking for dynamic staff who love to be challenged.
You will be working not only with the Primary Link GP referral Service but also with the countywide Bed Coordination Team.
Our service hours are 08:00-21:30 7 days a week and will be worked in shift patterns, this includes bank holidays.
Informal visits are welcome.
