Client seeks Registered Nurse for contract assignment starting ASAP-December 31 at their Corporate Health clinics in Florence & Covington, KY. Clinics see patients for Primary Care, acute episodic care, physicals, and wellness visits. RN will work both front and back office and duties will include greeting and rooming patients, taking vitals and patient history, patient testing, giving injections and immunizations, drawing blood, assisting the provider, scheduling appointments, answering phones, and scanning/filing. Clinic hours are Monday 8:00am-4:30pm in Florence, Tuesday 8:00am-3:00pm in Florence, Wednesday 8:00am-4:00pm in Florence, & Thursday & Friday 7:30am-4:30pm in Covington. Provider must have 2+ years of clinical experience to qualify for this assignment and must have strong phlebotomy skills, organizational skills, and be comfortable working alone with a provider. Epic EMR experience is also a plus; vaccinated providers are preferred. RN may also be asked to fill in at 3rd clinic located 8 miles away.

The Concurrent Review Nurse utilizes clinical skills to coordinate, document and communicate all aspects of the utilization/benefit management program. Applies critical thinking and knowledge in clinically appropriate treatment, evidence-based care and medical necessity criteria for appropriate utilization of services.This is a telework position. Qualified candidates may reside in any of the 9 regions: Jefferson, Two Rivers, SRT, Northern Bluegrass, Northeastern, Southern Bluegrass, Cumberland, The Lakes, or Eastern Mountain. Some travel to the Louisville office may be required for meetings and training.Consults and lends expertise to other internal and external constituents in the coordination and administration of the utilization/benefit management function.Gathers clinical information and applies the appropriate medical necessity criteria/guideline, policy, procedure and clinical judgment to render coverage determination/recommendation/discharge planning along the continuum of care. Utilizes clinical experience and skills in a collaborative process to evaluate and facilitate appropriate healthcare services/benefits for members including urgent or emergent interventions (such as triage / crisis support).Coordinates/Communicates with providers and other parties to facilitate optimal care/treatment.Identifies members who may benefit from care management programs and facilitates referral.Identifies opportunities to promote quality effectiveness of healthcare services and benefit utilization.

The Medically Complex Nurse Case Manager is responsible for assessing members through regular and consistent in person or telephonic contact to assess, plan, implement and coordinate all case management activities with members to evaluate the medical and psychosocial needs of the member to facilitate and support the member’s improved health. The Case Manager develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness. Services strategies policies and programs are comprised of network management and clinical coverage policies.This is a field-based position that requires routine regional in-state travel 80-90% of the time; use of personal vehicle is required. Qualified candidates must have valid KY driver's license, proof of vehicle insurance, and reliable transportation. Travel to the Louisville office is also anticipated for meetings and training.This position is assigned to the Salt River Trail region (Anderson, Breckenridge, Bullitt, Franklin, Grayson, Hardin, Henry, Larue, Marion, Meade, Nelson, Oldham, Shelby, Spencer, Trimble, Washington, and Woodford counties).
