The Admissions Coordinator is responsible for assisting with the admission of patients to the Hospital according to established policies and procedures, and processing admission paperwork and related records. Ensures accuracy and timeliness in required data collection and reporting to facilitate payment and regulatory compliance. Serves as a liaison with patients’ family or responsible party at time of admission and works closely with Hospital staff to ensure efficient admission process and transition. Consistently provides exemplary customer service to all patients, patient family members and Hospital staff

The Activities Director is responsible for promoting the well-being and functional independence of each resident, by developing, supervising and delivering the facility’s exercise and activities programs. Responsible for the evaluation of residents with a need for leisure services, and for the implementation of SNF/TCU activity services. Develops recommendations regarding functional outcomes and contributes to the interdisciplinary treatment plan. Implements and supervises the appropriate therapeutic interventions.

This position will act as part of the dual eligible member's care team at the state level responsible for integrating and coordinating care to meet the uniquely complex healthcare needs of our DSNP members. This position will assist our dually eligible members with maintaining Medicaid and LIS eligibility and obtaining other Medicaid entitlements available to them. They will be responsible for monitoring member statuses and reporting results to management. The employee must be able to work alongside other DSNP care team members including social workers, care managers and care coordinators to provide the member experience by: • Educating and assisting members on various elements of Medicaid entitlements, benefit plan information and available services created to enhance the overall member experience with the company. • Utilize all relevant information to effectively influence member engagement. • Initiating contact with members who have lost Medicaid eligibility and fall into DSNP grace period to obtain Medicaid and LIS certification. • Assist members with finding resources to help them reapply if necessary. • Coordinating annual reminders for members at risk of losing LIS, Medicaid or DSNP eligibility. • Taking ownership of each customer contact to anticipate customer needs, resolve their issues and connect them with additional services as appropriate. • Identifying member needs beyond the initial inquiry by answering the unasked questions and asking probing questions to identify member needs

This position is responsible for the medical leadership, clinical oversight and patient care, as well as medical leadership. It is a full time (5 day per week) position with on-call responsibilities. This position reports clinically to the Regional Medical Director and administratively to the Associate VP of Operations.Required QualificationsMinimum Educational requirement: MD or DO degree, Board Certified in Occupational Medicine, Family Practice or primary care specialty as defined by the American Board of Medical specialties.7-10 years minimum in Primary care, occupational medicine preferredMust have current state and federal DEA certification.Current certification in AHA or ARC Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services.Minimum 5 years medical management/leadership experience.Knowledge and experience with outcomes based health initiatives.Demonstrated exceptional interpersonal and collaboration skills.Superior verbal and written communication skills.Excellent presentation skills, confident in all settings with individuals at all levels of the organization both internal and external.Understanding of the economic trends in medicine.Excellent quantitative skills. Must be able to analyze data trends, and to critically analyze the medical literature.Demonstrated skill with Microsoft Office Suite and web-based programs.Licensed to practice medicine in the state within which the site is located.
