Essential FunctionsConsults with site health care professionals to determine patient's nutrition risk and needs.Conducts a comprehensive nutrition assessment, which includes obtaining a detailed nutrition history, review of risk factors, labs, and medical history, and identification of nutrition and cultural behaviors, readiness to change, barriers to change, and opportunities to build self-efficacy.Discusses results of nutrition assessment with patient identifying areas where patient can decrease health risks.Educates and guides patients in nutrition principles, food selection acquisition and preparation using National standards and guidelines.Develops a personalized nutrition prescription for each patient in collaboration with the patient’s PCP and Care Team.In a shared decision-making relationship, develops and documents patient centric goals and objectives with patient to meet the nutrition prescription.Develops and implements plan of care and correlates plan with other healthcare professionals.Evaluates nutrition prescription and provides follow-up as needed.Documents nutrition care plans and progress into patient's medical records.Identify patients for needed preventative and chronic condition services to assist in closing “gaps”.Participates as a member of the interdisciplinary care team.Works on a variety of projects- including program concept development, product/brand development, customer work sessions/team builds, culinary training, lifestyle medicine training, and other company and customer related projects as needed.Conducts professional and engaging presentations with confidence, ensuring seamless execution through advance planning, organization, thorough communication and appropriate time management practices.

Vacancies: 2Reporting to the Head Clinician, the nutritionist is responsible for conducting nutritional assessments, providing nutrition counseling, preparing nutrition care plans, and acts as the nutrition resource person for the hospital.In addition, she/he is expected to teach group sessions on issues related to nutrition and make nutrition-related presentations to interested groups in the hospital.The ideal candidate will be required to Manage Outpatient Therapeutic Program (OTP) and Food by Prescription (FBP) feeding programmes including carrying out research and development of new nutritional products to meet emerging clients' needs.
