MedSpan program provides HIV patient education, treatment adherence and discharge planning/connection to care services.Program staff work very closely with HIV/Infectious Disease Specialists and other interdisciplinary Correctional Health staff in all correctional facilities.Responsibilities• Proper documentation of all direct and indirect patients encounters in electronic health records utilizing specific MedSpan templates• Daily identification of new HIV rapid positive and Hepatitis C rapid positive patients via electronic health records reports manager to provide an intensive HIV health education and complete an initial evaluation utilizing initial patient visit templates.• Continual follow up of new HIV rapid positive patients throughout incarceration• Review HIV medication compliance list via electronic health records reports manager to identify patients who are not compliant with ARV medications and evaluate these patients for treatment adherence and patient education on a regular basis• Daily identification of Known HIV patients admitted in the last 25 4 hours to evaluate continuation of anti-retrovirals, required immunizations and referral to HIV Specialists• Contact community providers to confirm proper anti-retroviral regimen• Discharge planning and connection to care – track discharge dates of HIV patients, arrange for post discharge medical ow-up in the community, carry meds, prescriptions and confirm if patient kept their outside appointment (connection to care)• Monitor patient labs to identify any significant changes in viral load and CD4 to identify non-adherence or treatment failure• Evaluate HIV negative patients on post exposure prophylaxis (PEP)• Cross cover other facilities to provide medspan services• Complete and update HIV comprehensive and connection to care databases on a continual basis• Collaboration with HIV Specialist and other correctional health staffAttend required trainings and continuing education
