Provides intensive clinical services to consumers and families in their homes throughout Chambers County. Develops consumer plans and oversees their implementation. Provides clinical consultation to other staff and agencies within the community in accordance with professional qualifications and credentials. Therapeutic Mentoring Services provide a structured one on one intervention to a child or youth and their families that is designed to ameliorate behavioral health-related conditions that prevent age-appropriate social functioning. This service includes supporting and preparing the child or youth in age appropriate behaviors by restoring daily living, social and communication skills that have been adversely impacted by a behavioral health condition. These services must be delivered according to an individualized treatment plan and progress towards meeting the identified goals must be monitored and communicated regularly to the clinician so that the treatment plan can be modified as necessary. Therapeutic mentoring may take place in a variety of settings including the home, school or other community settings. The therapeutic mentor does not provide social, educational, recreational or vocational services.

Requests and identifies all necessary orders so that they are available to the therapy staff for the stated diagnosis. Also, assures that the patient is scheduled appropriately for that diagnosis. Communicates with physician offices, patients and ancillary departments regarding scheduling procedures, appointments and coordination of tests. Responsible for insurance verification of therapy benefits and following the pre-certification guidelines to obtain and complete authorization. Is responsible for the delivery of quality performance and service excellence; and adheres to the Parkview Health Standards of Behavior.
