All services are meticulously documented in compliance with clinic policies and procedures, while maintaining utmost confidentiality and privacy. Additionally, the Social Medicine Physician offers resources, referrals, and support to patients as needed, ensuring their holistic well-being. General Responsibilities: 1. Conduct interviews, screenings, and assessments to determine health, mental health, substance use, and social determinants of health needs 2. Provide psychoeducation, case management, coordination, and brief intervention services based on identified needs 3. Collaborate, coordinate, and consult with other professionals and staff to meet patient needs 4. Document all services in compliance with clinic policies and procedures 5. Offer services, resources, and referrals for patients as required 6. Maintain clinical documentation as specified by department's compliance policies, update patient records as needed, and protect private information
