General Responsibilities: 1. Develop and monitor care management and care coordination processes 2. Support primary clinical teams with the abovementioned efforts 3. Identify the high acuity patient population and work more directly to ensure care management, as well as care coordination for this patient population 4. Serve as a resource on care management for all patients across clinical teams 5. Consult with patients and family members to discuss their health problems 6. Educate patients about their condition, medication, and provide specific instructions 7. Develop personalized care plans to address patients' healthcare needs 8. Consult and collaborate with other healthcare providers and specialists to schedule patient appointments and create treatment plans 9.
