Hospice and Palliative Medicine

Hospice and palliative medicine physicians have specialized knowledge and skills to prevent and relieve the suffering experienced by patients with life-limiting illnesses. Both palliative care and hospice care provide comfort, but palliative care can begin at diagnosis and alongside treatment. Hospice care, on the other hand, starts after treatment is stopped and when it is clear that the person will not survive the illness. Physicians specializing in hospice and palliative medicine work collaboratively with other healthcare professionals, actively listen to patients, align treatments with their priorities, and assist families in navigating the complex healthcare system. General Responsibilities: 1. Develop a personalized care plan for pain management and symptom relief, providing necessary palliative drugs, therapies, medical supplies, and equipment. 2. Deliver comfort in the patient's own home, with a family member acting as the primary caregiver under professional medical supervision. 3. Assess individual needs and offer additional care and services, such as speech and physical therapy, therapeutic massage, or dietary assistance. 4. Address the psychological, social, and spiritual needs of the patient. 5. Work towards avoiding avoidable admissions towards the end of life. 6. Support anticipatory planning with patients and professionals, including advance planning in the last year of life. 7. Ensure end-of-life care that aligns with the individual's and family's needs and wishes. 8. Provide support to the family and significant others during the patient's illness and their bereavement.
