Health Records Assistant
Description:
A Health Records Assistant is responsible for documenting patients’ health and personal information in stipulated files for record keeping and maintaining the files included in a patient's health information portfolio. They are responsible for organizing and managing patients' health information data, ensuring that all documents are completely and accurately filled and verified. They collect patients’ demographic data for easy identification and contact in cases of emergency and correspondence. Additionally, they communicate with physicians and other healthcare professionals to clarify diagnoses or obtain additional information. They also maintain electronic health records (EHR) databases, analyze electronic data, and maintain data on patient safety, patterns of disease, and disease treatment and outcome. They work to improve upon EHR software usability and develop and maintain health information networks. They also carry out periodic reviews of patients’ medical records to ensure that old and outdated information is updated to meet the current status. Overall, a Health Records Assistant plays a crucial role in ensuring the accuracy and accessibility of patients' health records.
General Responsibilities:
1. Organize and manage patients' health information data
2. Ensure that all documents are completely and accurately filled and verified

14. Make patients’ charts and records available to relevant department in accordance with clinic procedures
15. Compile both new and old medical records of clients for clarification and comprehension
