Social Worker

Social Workers are professionals tasked with improving people’s overall wellbeing, particularly the most vulnerable populations. They are responsible for areas of care management and assist patients and families in understanding patient conditions and treatments. Social Workers provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. They help patients and family members develop coping strategies, assess patient needs, assist in the development of treatment plans, and provide emotional and mental assessments on the patient. Additionally, they offer important services aimed at promoting healthy lifestyles, preventing diseases, and addressing barriers to access. General Responsibilities: 1. Cooperate with other professionals to evaluate patients' medical or physical condition and to assess client needs 2. Collaborate with other professionals to assess client needs or plan treatments 3. Investigate child abuse or neglect cases and take authorized protective action when necessary 4. Interview clients to gather information about their backgrounds, needs, or progress 5. Plan and conduct programs to combat social problems, prevent substance abuse, or improve community health and counseling services 6. Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the client or patient 7. Identify environmental impediments to client or patient progress through interviews and review of patient records 8. Perform a comprehensive assessment of the patient’s social, emotional, environmental, and financial needs 9. Help patients and their families cope with the emotional and social responses to illness and treatment 10. Educate patients and their families on entitlements, community resources, and health insurance coverage 11. Ensure that the services the patient requires are in place and that the patient will be properly cared for at home
